Registered Nurse; RN - Home Health - per visit

Job Overview

BAYADA Home Health Care is seeking a Registered Nurse (RN) to provide home health visits in Emmaus, Pennsylvania, serving adult and geriatric patients throughout Lehigh County. The position offers per diem, part‑time, or full‑time opportunities with flexible schedules and a 1:1 care focus.

• Follow a designated care plan in accordance with patient needs.
• Make home visits to clients within assigned geographic territories.
• Administer medication, wound care, treatments, and procedures.
• Monitor clients’ conditions and report changes to the Clinical or Client Services Manager.
• Execute and properly document doctors’ orders.
• Perform client assessments as necessary.
• Coordinate case management and service delivery.
• Accurately document observations, interventions, and evaluations using a state‑of‑the‑art tablet interface.

• Current license as a Registered Nurse in Pennsylvania.
• Minimum one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred but not required.
• Graduate from an accredited nursing program, evidenced by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal and communication skills.
• Solid computer skills; prior experience with electronic medical records preferred.
• Willingness to travel to cases as assigned.
