RN - Registered Nurse | Horizon Home Health | Emmett

Full‑time Home Health RN position in the Emmett, Idaho area. The role requires a compassionate RN with strong clinical skills and a desire to be part of an excellent team.

• Registered Nurse (RN) licensed in Idaho.
• Valid driver’s license and auto insurance.
• Experience in home health preferred.

• Complete initial assessment of patients and families to determine home care needs.
• Provide comprehensive assessment and history of current and previous illnesses.
• Regularly re‑evaluate patient nursing needs.
• Initiate and revise plan of care based on patient status.
• Use assessment data to determine nursing diagnosis.
• Develop care plan establishing goals and incorporating therapeutic, preventive, and rehabilitative nursing actions, including patients and families in planning.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed.
• Counsel patients and families to meet nursing and related needs.
• Provide health care instructions per assessment and plan of care.
• Identify discharge planning needs and implement prior to patient discharge.
• Act as case manager when assigned by Clinical Supervisor, coordinating care for assigned caseload.
• Adhere to organization’s documentation and care procedures and maintain professional conduct.

• Prepare clinical notes and update the primary physician as necessary, at least every sixty days.
• Communicate with physician regarding patient needs and report changes in condition; obtain and receive physician orders as required.
• Communicate with community health related persons to coordinate care plan.

• Participate in on‑call duties as defined by the on‑call policy.
• Ensure arrangements for equipment and other necessary items and services are available.
