White Sands, a Human Good life plan community (CCRC) in beautiful La Jolla, CA, is seeking a Social Services Coordinator to join our team. Under the supervision of the Social Worker, the Social Services Coordinator organizes, implements, and evaluates the Social Services program to meet the psycho-social needs of our residents. This includes, but is not limited to, maintenance of resident records, coordination of services, and relocation when necessary. As a representative and team member of the company, this position is expected to present oneself in a manner that reflects professionalism and ensures resident satisfaction.
Full-time:
Monday - Friday (9am-5pm)
Pay Range: $23 - $26/hour, based on education and experience
To be successful in this role, you would have:
- Bachelor's degree in social work or related program such as gerontology, psychology, or recreation therapy.
- 1-2 years’ experience in case management required, preferably in a geriatric setting or any equivalent combination.
- 1+ years' experience working with older adults required
- Social Services certification preferred.
Job Duties:
- Assesses resident needs based on social services history, resident's health, psychological and social needs;
- Develops a Resident Care Plan based on assessments;
- Visits and interviews residents at quarterly intervals and when requested by staff, physicians, residents, or family members to determine recreational/activity history, needs, and interests, as well as social services' needs;
- Counsels and assists families with issues or concerns relating to caring for a resident;
- Completes documentation regarding social service needs as required by regulations;
- Coordinates, facilitates and organizes educational programs for residents regarding health, safety, and quality of life concerns;
- Provides general case management (including move in) and referral services to all residents needing such assistance;
- Educates residents and families on service availability, application procedures, client rights, etc., providing advocacy as appropriate;
- Works with residents, families and outside agencies when need for a higher level of services has been identified;
- Refers and helps residents schedule appointments with the community's local service providers;
- Establishes relationships with individual residents, their families and neighborhood groups;
- Promotes and protects the rights of each resident
