Description
Johnson Memorial Hospital is looking for LMSW Social Worker / Discharge Planner to join our Case Management team. The Social Worker is responsible for assessing and providing direct Social Work services and interventions to patients and families in the hospital setting upon admission and throughout discharge.

You Will Be Responsible For- Provide ongoing case management support to the patients.
- Assess patient's needs and develop discharge care plans.
- Communicate effectively with staff, colleagues, patients, and family members.
- Utilize standard Social Work intervention techniques collaborating with patients and families.
- Work as a resource to other staff members.
- MSW degree from an accredited School of Social Work required.
- CT State Social Work Licensure LMSW preferred.
- Certification in case management (CCM) and certification in Continuity of Care – advanced (ACCC) preferred.
- Knowledge of Medicare, Medicaid, and managed care regulations and trends.
- Experience with utilization management, home care, community resources, and rehabilitation beneficial.
- Engaged supportive Leadership.
