Trinity Health Of New England is seeking an Oncology Nurse Navigator (RN) to guide oncology and hematology patients through all stages of treatment.
Location:
142 Hazard Ave, Enfield, CT.
Hours:
8:00am - 5:00pm, Monday–Friday.
You will advocate for patients, coordinate care, educate families, and ensure access to high-quality cancer care. Responsibilities include coordinating care plans, educating patients, documenting in Epic EMR, and supporting quality and compliance initiatives such as MIPS
