We are seeking a compassionate and highly organized
** Oncology Nurse Navigator (RN)
** to guide oncology and hematology patients through every stage of their treatment journey. In this role, you will serve as a patient advocate, care coordinator, educator, and clinical resource, ensuring timely access to high-quality, patient-centered cancer care.
** Key Responsibilities*
* + Coordinate care plans, appointments, treatments, referrals, and follow-up services for oncology and hematology patients.
+ Assess patient needs and collaborate with providers to ensure safe, evidence-based care.
+ Educate patients and families on diagnoses, treatment plans, symptom management, and available resources.
+ Monitor patient progress, document outcomes in
** Epic EMR** , and communicate changes in condition to providers.
+ Facilitate chemotherapy scheduling, treatment authorizations, and coordination with pharmacy and clinical teams.
+ Connect patients with support services, transportation, financial resources, survivorship programs, and clinical trial opportunities.
+ Ensure medical records and treatment information are available for all patient visits.
+ Support quality, compliance, and reporting initiatives, including
** MIPS, QOPI, CoC, and Medicare oncology program requirements** .
+ Develop partnerships with community organizations and patient support groups to enhance patient access to care.
** Qualifications*
* + Graduate of an accredited nursing program;
** BSN preferred.*
* + Current
** Connecticut RN license
** in good standing.
+ Current
** CPR certification** (or ability to obtain).
+ Experience in
** oncology, hematology, infusion, navigation, or care coordination
** preferred.
+ Strong clinical assessment, patient education, communication, and organizational skills.
+ Proficiency with electronic medical records; experience with
** Epic
* * preferred.
+ Ability to thrive in a fast-paced environment while providing compassionate patient-centered care.
