Occupational Therapist - Home Health

Klarus Home Health is seeking a full time, part time or PRN / Per Diem Licensed Occupational Therapist to join our team to provide OT services including assessment, treatment planning, implementation, and evaluation for patients enrolled in our home health program in the North Denver/Denver Proper regions and surrounding areas.

• Conduct clinical assessments and OASIS assessments to identify patient needs and determine levels of functioning.
• Evaluate patients’ occupational performance in areas such as self-care, work, leisure, and rest.
• Assess sensory-motor neuromuscular, psycho-social, and cognitive-perceptual components of occupational behavior.
• Evaluate physical, social, and cultural components of patients’ occupational environment.
• Facilitate direct therapeutic interventions to address range of motion, strength, coordination, and adaptive equipment needs; implement environmental modifications as needed.
• Develop, evaluate, and revise the plan of care in collaboration with patients, representatives, and caregivers.
• Document clinical notes, phone orders, and other records in a timely manner.
• Accept clinical assignments consistent with education and competence; provide physician-ordered services including counseling and education for patients, caregivers, and families.
• Supervise and guide occupational therapy assistants and home health aides as appropriate.
• Apply infection control and standard precautions to prevent contamination and transmission of disease.
• Utilize effective interpersonal relations and communication skills to achieve desirable outcomes.

• Education:
  
  Bachelor of Science in Occupational Therapy.
• Licensure:
  Current State License as an Occupational Therapist; current driver’s license required.
• Experience:
  
  One year of experience as an Occupational Therapist preferred; home health experience preferred.
• EMR:
  Home care HHC base proficiency preferred.
• Skills:
  
  Proficiency in therapy skills per accepted standards of practice, good interpersonal skills, current CPR certification, and knowledge of durable medical equipment.

  Work in patients’ homes under various conditions; possible exposure to blood, bodily fluids, and infectious diseases; flexible schedule with local travel; some exposure to unpleasant weather conditions.

• Ability to perform heavy lifting, bending, pulling, pushing, and prolonged standing/walking.
• Capability to work under stressful conditions to meet deadlines and patient needs.
• Hand-eye coordination and manual dexterity.
