Description

Klarus Home Health is looking for a full‑time, part‑time, or PRN Licensed Practical Nurse (LPN/LVN) to join our dedicated team to provide community‑based care for our patients currently receiving home health services in the Englewood, Aurora, Lakewood, and/or Denver metro area.

The primary function of the LPN is to provide nursing care under the supervision of a registered nurse for patients of all ages in their place of residence. The LPN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes.

Key Responsibilities
  • Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse.
  • Coordinate care with the interdisciplinary group, patient/family, and referring agency.
  • Assist in the development and implementation of patient care plans.
  • Monitor patient health and report any changes to the supervising RN.
  • Administer medications and treatments as prescribed.
  • Educate patients and their families on health care needs, conditions, and options.
  • Maintain accurate and timely documentation of patient care and progress.
Job Qualifications
  • Education:

    Graduate of an accredited School of Vocational/Practical Nursing.
  • Licensure:
    Current State license as a Licensed Vocational or Practical Nurse, or a multi‑state license issued by a state member of the NLC if practicing in a state member of the NLC.
  • Must have a current driver’s license.
  • Experience:

    Two years’ experience as an LPN in a clinical care setting. Home health experience highly preferred.
  • EMR system: HCHB experience highly preferred.
  • Skills:

    Proficiency in nursing skills as defined by generally accepted standards of practice; good interpersonal skills.
  • Transportation:
    Reliable transportation and valid, current auto liability insurance.
Environmental And Working Conditions
  • Work in patients’ homes in various conditions.
  • Ability to work a flexible schedule and travel locally.
  • Ability to lift up to 50 lbs and move patients.
