Pay Range: $19.00 - $21.00

Shift: 2pm - 10pm (Schedule to be discussed at interview at the community)

Certification Requirement: Must have a valid QMAP certification in CO.

Job Summary

The Med Care Manager/QMAP must administer, pour, pass, and document all routinely prescribed medications per physician orders under the direction of the Wellness Director and as approved by the State Medication Administration Regulations. You will quickly learn each resident’s medication preferences and habits.

Position Overview
  • Administer and document regularly prescribed medications per the Medication Administration record, observing the five rights and physician orders.
  • Order meds from the pharmacy as prescribed, 7 days prior to running out, and as requested by the Wellness Director.
  • Immediately after administering medication, document the medication given in the resident’s medical record and date/initiate the bubble pack.
  • Observe and report to the licensed nurse any reactions or side effects.
  • Take and record vital signs before administering medications that may affect or change vital signs.
  • Administer PRN medications per physician order.
  • Report any medication error to the Wellness Director immediately.
  • Assist with development and review of the resident’s care plan in conjunction with other disciplines.
  • Ensure residents’ privacy, respect, and dignity.
  • Detect, correct, and report unsafe conditions that may result in harm.
  • Assure physical comfort, safety, and mental well‑being of residents.
  • Assist in daily requirements and tasks such as toileting, grooming, bathing, feeding, dressing, monitoring intake, caring for hair and nails, moving residents, responding to calls, etc.
Qualifications

  • License/Certification:
    Completion of state‑approved Medication Administration training, First Aid certification per ALF regulations, QMAP in CO.
  • Education/

    Experience:

    Minimum high school diploma or equivalent; preference for some healthcare experience, prior work in Assisted Living or Memory Care, experience administering medication in assisted living, and a record of good work.
  • Continuing education:
    Attend monthly in‑service education and training within the community.
  • Other:
    Ability to read and write English; read and interpret documents such as safety rules, operating and maintenance instructions, procurement manuals, etc.; good communication skills; ability to make decisions with residents’ interests at heart.
  • Job Knowledge:
    Execute medication administration functions and understand required procedures; knowledge of universal precautions.
  • Industry

    Experience:

    Minimum three years in senior living preferred; passion for working with seniors; previous experience in an Assisted Living community.
