Job Summary:
Preventel Health is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team of clinical experts in Arapahoe County, Colorado. As an LPN – Hybrid Clinical & Care Management professional, you will have the opportunity to provide exceptional patient care and exceptional quality in every detail. If you are passionate about delivering personalized care and are willing to travel to various facilities, we encourage you to apply for this full-time position.
Responsibilities:- Communicate effectively with patients, family members, and healthcare professionals to ensure seamless care coordination
- Maintain accurate and detailed records of patient care, treatment plans, and medication administration
- Travel to various facilities in the state of Colorado to provide care and support
- Must have an up-to-date LPN degree in the state of Colorado
- Must be willing to travel to various facilities in the state of Colorado
- Must have Nursing Home experience for at least 1 year (Please do not apply if you do not have this)
- Must be personable, as this is a concierge, client-facing job
- 1 year of minimum experience as an LPN
- Competitive hourly rate: $26 - $30 per hour
