Family Tree House of Hope is a 90‑day emergency homeless shelter dedicated to providing a safe and supportive environment for families with children experiencing homelessness. HOH provides advocacy services and case management to families with children. Family Tree’s House of Hope operates 24/7.
Position PurposeSupport clients' needs by connecting them to various resources for housing, mental health, career development, housing options, etc. Being knowledgeable and up to date on available resources, advocating for clients, and coaching clients on how to support themselves—implementing the programming, documenting important client information through program data management, and case notes—working from a trauma‑informed approach and Family Tree's equity lens to ensure a client‑centered mindset.
EssentialDuties / Responsibilities
- Orient new clients to shelter and explain house guidelines.
- Provide information, referral, and advocacy to current and prospective clients.
- Assist clients practicing appropriate coping skills and safety planning.
- Complete documentation in daily communication log entries, weekly case notes, HMIS, and other written information regarding client issues.
- Assess clients' needs and develop individualized case plans for self‑sufficiency, which may include assisting with benefits acquisition, educational/vocational referrals, financial empowerment, behavioral/health referrals, goal setting, life skills, and the establishment of a personal support network.
- Complete daily safety checks for each room.
- Meet with clients weekly to discuss their case plan, progress, housing options, and other issues.
- Coordinate case services with other agencies to meet client needs.
- Complete intakes with new clients and exit paperwork with outgoing clients.
- Provide advocacy, crisis intervention, and conflict resolution to households who have experienced domestic violence.
- Maintain confidentiality of shelter, employees, and client information per Family Tree policies, state and federal laws.
- Communicate with partner agencies and other professionals as needed.
- Adhere to all Family Tree, Inc. established policies and procedures, including those regarding timely reporting of suspected child abuse or neglect and confidentiality of client information.
- Attend supervision twice per month with the Program Director to communicate needs and concerns as they arise.
- Assist in training and directing interns, volunteers, and relief staff.
- Attend and participate in weekly staff meetings by presenting cases and offering input to other cases, including case coordination and case plan implementation.
- Maintain regular attendance at a reliable level.
- Mediating issues between clients in the shelter.
- Plan groups and activities for clients that are held by other community agencies or by the CM themselves.
- Adhere to Family Tree's Continuous Improvement Practice by completing client support tools and sending feedback and post‑exit surveys to clients according to Family Tree's standards.
- Serve as a support to volunteers, interns, and other staff while at Family Tree's House of Hope.
- Communicate and coordinate with the Program Director and Assistance Director for materials and items needed.
