MJHS Hospice and Palliative Care is seeking a Nurse Practitioner to join our team for per diem coverage. The NP will staff consultations in nursing homes, hospitals or community settings, coordinating with an interdisciplinary team to develop care plans and ensure high-quality end-of-life care.
The role requires NYS RN licensure and NP certification, with strong leadership, communication and documentation skills. Per diem visits and visits to community cases are included in coverage areas.
