Responsibilities:- Provide direct patient care and support in accordance with your licensure and certification level
- Assist patients with activities of daily living (ADLs) including bathing, grooming, and mobility
- Monitor and document patient health status, vital signs, and changes in condition
- Administer medications and treatments as directed (RN/LPN)
- Collaborate with care teams to develop and implement individualized patient care plans
- Offer emotional support and companionship to patients and their families
- Maintain a safe, clean, and comfortable environment for patients
- Active and valid RN, LPN, HHA, PCA, or CNA license or certification in New Jersey
- Prior experience in home health, assisted living, or clinical care preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a collaborative care team
- Reliable, punctual, and committed to delivering high-quality patient care
- Compassionate, patient-centered approach to caregiving
- Ability to pass a background check and meet all state compliance requirements
