Assistant Director of Nursing (ADON) / MDS Nurse (RN)

We are seeking an experienced and compassionate ADON / MDS Nurse (RN) to join our leadership team. This dual-role position supports the Director of Nursing with daily clinical operations while overseeing the MDS and care planning process to ensure quality resident care and regulatory compliance.

• Assist with daily nursing department operations and staff supervision
• Ensure compliance with state and federal regulations
• Coordinate MDS assessments and interdisciplinary care plans
• Lead care plan and Medicare/Utilization Review meetings
• Monitor resident care, assessments, and documentation accuracy
• Support QAPI and performance improvement initiatives
• Conduct nursing rounds and staff education as needed
• Collaborate with therapy, nursing, and interdisciplinary teams

• Current RN license in practicing state
• Minimum 2 years of MDS experience in long-term care
• Supervisory or leadership experience preferred
• Strong organizational and communication skills
• Knowledge of MDS, Medicare, and long-term care regulations
• CPR Certification required
