About This Role

The Senior Care Manager (RN) assesses, plans, and implements complex care management activities to enable quality, cost-effective healthcare outcomes. This role is ideal for experienced nurses who excel in communication and have a background in managing complex medical caseloads.

• Develop and continuously assess ongoing care plans/service plans.
• Collaborate with providers to identify needed resources for members.
• Coordinate care between members, families, and care provider teams.
• Monitor care plans and member status, revising as necessary.
• Identify problems/barriers for care management in escalated cases.
• Review member data to identify trends and improve care quality.
• Develop appropriate care plans/service plans from referrals and assessments.
• Perform outreach to assess member needs and collaborate with resources.
• Collect, document, and maintain member information for compliance.
• Provide education to members and their families on healthcare options.

• Education: Degree from an Accredited School of Nursing or Bachelor's degree in Nursing required.
• Experience: 4 – 6 years of related experience required.
