Job Description

We are seeking an experienced Licensed Practical Nurse to join our home health team doing weekend visits! You will be responsible for the assessment and treatment of assigned patients, based on agency policies and procedures, through the competent application of the nursing process.

Responsibilities:

• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating members of the interdisciplinary team
• Educate patients about health maintenance and disease prevention
• Maintain accurate patient medical records
• Provide advice and emotional support to patients and their family members
• Provides services requiring substantial and specialized nursing skill, in accordance with the plan of treatment signed by the physician and makes the initial evaluation visit to the client.
• Demonstrates competency in all skills required for the agency, including, but not limited to, venipunctures, infusion therapy, infusion therapy device care, wound care, and aseptic technique.
• Performs client assessments based on agency standards of practice to ensure effective and appropriate home care services.
• Submits accurate documentation within 24 hours of visit.
• Participates in on-call coverage as needed

Qualifications:

• Previous experience in nursing
• Familiarity with EMR systems
• Ability to build rapport with patients
• Strong problem solving and critical thinking skills
• Ability to thrive in a fast-paced environment
• Active, unencumbered LPN license in Oklahoma required
• Current CPR certification preferred

Experience Requirements

• Minimum six months nursing experience
• Home Health Experience preferred
