Licensed Practical Nurse (LPN)

Location: Tulsa, OK

Schedule: Monday‑Friday 5:00 am‑5:00 pm (Weekday and Weekend shifts available).

Aveanna’s Private Duty nurses assist patients in transitioning home from the hospital and provide continuous care so patients can remain at home with loved ones. We match nurses to patients based on skill, age, health condition, and acuity level.

Responsibilities
  • Implement physician‑prescribed plan of care.
  • Perform patient assessments and coordinate care.
  • Administer prescribed medication, treatments, and therapies.
  • Comply with timely documentation of care in the system of record.
  • Promote health, teach, and train family members.
  • Accept and write verbal orders from physicians as required; obtain co‑signature of Case Manager for all verbal orders.
  • Follow up with physician, client, family members, staff, and Case Manager on client problems, concerns, or changes in condition.
  • Perform other appropriate duties as assigned.
Qualifications
  • Active, unencumbered license (RN/LPN/LVN) in the state where the clinician will practice.
  • Current CPR certification from American Red Cross, American Heart Association, or BLS.
  • Current TB skin test within last 12 months.
  • Preferably six months of hands‑on nursing experience (not required).
  • Reliable transportation.
