RolePurpose
You are the clinical leader, advocate, and steady guide for your patients and their families, leading the plan of care, managing symptoms, and ensuring personalized, comfort-focused care aligned with goals and wishes. You bring clinical expertise, reassurance, clarity, and compassion to each visit.
Responsibilities- Lead Patient-Centered Hospice Care:
Conduct comprehensive patient and family assessments;
Develop and manage individualized, comfort-focused care plans;
Deliver skilled nursing care with a focus on pain and symptom management - Serve as the Care Coordinator:
Act as case manager, coordinating care across the interdisciplinary team;
Collaborate with physicians, social workers, chaplains, and aides;
Monitor patient condition and adjust care plans to maintain comfort - Support Patients & Families:
Provide education, emotional support, and guidance;
Help families navigate complex decisions with empathy and clarity;
Build meaningful, trusting relationships - Drive Quality & Compliance:
Maintain accurate, timely documentation;
Participate in IDT meetings and care planning;
Support quality and compliance standards
- Active RN license (or compact eligibility)
- Graduate of an accredited nursing program
- Minimum 1 year of clinical nursing experience
- Strong clinical judgment and compassionate care approach
- Ability to manage a caseload independently
- Excellent communication skills in sensitive situations
- Basic EMR proficiency
- Valid driver’s license and reliable transportation
- Experience in hospice, palliative care, or home health
- Strong pain and symptom management skills
- Case management and interdisciplinary care experience
- Ability to quickly build trust with patients and families
- Experience with Homecare Homebase (HCHB)
- Field-based care in homes, facilities, and community settings
- High autonomy with strong team support
