Position Overview:

We are seeking a dedicated and compassionate Community Health Worker to join our team in Enterprise
. This position operates within a
Certified Community Behavioral Health Clinic (CCBHC) model, supporting individuals and families in navigating behavioral health, medical, and community-based services through a person-centered, integrated care approach. This role plays a vital part in improving access to care, supporting service coordination, and helping consumers successfully navigate the healthcare system to promote wellness, recovery, and continuity of care.

Qualifications:
  • Must have a High School Diploma or GED
  • Strong understanding of healthcare delivery systems and the importance of integrated, whole-person care
  • Experience or familiarity with care coordination processes, including transitions between providers, services, and levels of care
  • Strong communication, advocacy, and relationship-building skills
  • Ability to work effectively with individuals, families, healthcare providers, and community partners
  • Proficiency in health IT systems and electronic documentation platforms preferred
  • Additional certification in case management, community health work, or a related field highly desirable
  • Knowledge of behavioral health services, community resources, and person-centered care principles preferred
Responsibilities:
  • Support coordination of care across behavioral health, primary care, social services, and community-based providers to ensure consumers receive comprehensive, continuous services
  • Assist consumers and families in understanding and navigating the healthcare system, available services, benefits, and community resources
  • Help facilitate smooth transitions between levels of care, providers, and support systems to promote continuity and reduce barriers to treatment
  • Collaborate with multidisciplinary team members including primary care providers, behavioral health clinicians, care coordinators, and social service agencies to support individualized care planning within the CCBHC care model
  • Facilitate communication between consumers, families, and providers to ensure services align with individual needs, preferences, and recovery goals
  • Maintain accurate, timely, and confidential documentation of care coordination activities in compliance with agency standards, privacy regulations, and CCBHC requirements
  • Stay informed on best practices in care coordination, integrated care, and community health support through ongoing training and professional development
  • Advocate for consumers and families by helping reduce barriers to healthcare access, treatment engagement, and community support services
Requirements

Must have High School Diploma or GED with solid computer skills and one year or paid or volunteer experience in community health, medical records, call-center patient support or quality assurance work.

ALL candidates must be able to type and have basic computer skills, possess a valid driver’s license, eligibility for coverage under center liability insurance and individually held vehicle liability insurance with a clear background check.
