We are seeking a dedicated and detail-oriented Community Health Worker – Records Specialist to join our team. This position operates within a Certified Community Behavioral Health Clinic (CCBHC) model, supporting whole-person and integrated care through health information management, care coordination, and accurate clinical documentation. In this role, you will work primarily behind the scenes to review consumer records, track referrals, obtain outside medical records, identify documentation gaps, and help ensure necessary health information is available to support continuity of care.
QualificationsMust have a High School Diploma or GED
; additional education or qualifications in healthcare or social services preferred
Knowledge of behavioral health, integrated healthcare, and Social Determinants of Health (SDOH)
Understanding of CCBHC standards and care coordination principles
Strong organizational skills with excellent attention to detail
Strong computer skills, including Microsoft Office and electronic health record systems
Ability to communicate professionally with hospitals, physician offices, laboratories, pharmacies, and other healthcare providers
Ability to maintain accurate documentation and tracking systems while managing multiple priorities
Ability to work independently and collaboratively with multidisciplinary treatment teams
Must have a current driver’s license

Review consumer records to ensure completion of required CCBHC screenings, referrals, medical information, and documentation
Identify missing or incomplete records and follow up to obtain necessary health information
Retrieve medical records through Alabama One Health Record (ALOHR) and other approved health information exchange systems
Request and obtain records from hospitals, physician offices, laboratories, pharmacies, and other healthcare providers
Upload, organize, and accurately maintain external medical records within the electronic health record
Review records for emergency department visits, hospital admissions and discharges, crisis encounters, and other significant healthcare events
Notify therapists, care coordinators, nurses, and other appropriate clinical staff of significant findings or identified care gaps requiring follow-up
Maintain tracking systems for referrals, outstanding medical record requests, and documentation deficiencies
Collaborate with multidisciplinary treatment teams to promote integrated healthcare and continuity of care within the CCBHC care model
Assist with quality assurance activities related to CCBHC documentation and compliance
Maintain accurate, timely, and confidential documentation while protecting the integrity of all protected health information
RequirementsMust have High School Diploma or GED with solid computer skills and one year or paid or volunteer experience in community health, medical records, call-center patient support or quality assurance work.
ALL candidates must be able to type and have basic computer skills, possess a valid driver’s license, eligibility for coverage under center liability insurance and individually held vehicle liability insurance with a clear background check.
