Shift: 6:45 AM – 7:15 PM
Position Summary
The Licensed Practical Nurse (LPN) is responsible for supervising and coordinating the day-to-day nursing activities within the facility while ensuring residents receive the highest quality of care. This role works closely with the nursing leadership team to maintain compliance with all federal, state, and local regulations and supports the overall health, safety, and well-being of our residents.
Key Responsibilities
- Supervise daily nursing activities and nursing personnel assigned to the unit
- Ensure quality resident care is provided in accordance with individualized care plans
- Monitor resident conditions and promptly report changes to the appropriate healthcare professionals
- Administer medications and treatments as prescribed
- Maintain accurate and timely clinical documentation
- Ensure compliance with federal, state, and facility regulations and standards
- Collaborate with physicians, nurses, residents, families, and interdisciplinary team members
- Support and mentor nursing assistants and other direct care staff
- Assist the Director of Nursing and Assistant Director of Nursing in maintaining excellence in resident care
Qualifications
Graduate of an accredited Nursing Program or approved Licensed Practical/Vocational Nursing program
Current, active, and unencumbered LPN license in the state of practice
Minimum of one (1) year of experience in a hospital, long-term care facility, skilled nursing facility, or related healthcare setting preferred
Strong clinical, organizational, and communication skills
Commitment to delivering compassionate, resident-centered care
Ability to work effectively in a team-oriented environment
