You will assess, plan, and implement nursing care aligned to individualized plans of care, coordinating with physicians, social workers, chaplains, and other team members to ensure compassionate, high-quality end-of-life care.
You will educate patients and families, document care activities, and participate in on-call and scheduling initiatives to support timely visits and effective care transitions.
