LPN Personal Care

Fairmount Homes, Inc, a nonprofit senior living community, is seeking an LPN Personal Care team member to provide compassionate, high-quality care to our residents. In this role, you will deliver hands-on personal care, administer medications and treatments, monitor resident conditions, and accurately document in our healthcare systems. You will collaborate with an interdisciplinary team to support care plans and promote resident dignity, comfort, and independence.

Responsibilities

• Provide hands-on personal care and LPN-level clinical support to senior residents.
• Administer medications and treatments per provider orders and facility policies.
• Monitor, document, and report changes in resident condition using EHR systems.
• Support care plans in collaboration with RNs, therapists, and interdisciplinary team members.
• Assist residents with ADLs, mobility, and comfort measures while promoting dignity and independence.
• Follow infection control and safety protocols to maintain a secure environment.
• Communicate respectfully with residents and families, supporting education and care decisions.
• Contribute positively to Fairmount Homes' community-focused, nonprofit mission.
