Licensed Practical Nurse

Fairmount Homes, Inc., a nonprofit senior living community, is seeking a compassionate Licensed Practical Nurse to join our Healthcare & Medical Services team. The LPN provides hands-on nursing care to residents, including medication administration, treatments, wound care, and monitoring of health status. Working within a supportive, community-focused environment, you will collaborate with RNs and an interdisciplinary team to implement care plans, uphold infection control and safety standards, document in electronic health records, and advocate for residents' dignity, comfort, and well-being every day.

Responsibilities

• Provide direct bedside care to senior residents in a long-term care setting
• Administer medications and treatments in accordance with physician orders and LPN scope
• Monitor and document vital signs, resident status, and responses to treatment in EHRSupport RNs and interdisciplinary team in developing and implementing care plans
• Communicate changes in resident condition promptly to RN, providers, and families
• Maintain infection control and safety standards in all resident care activities
• Assist with admissions, transfers, and discharges, ensuring accurate documentation
• Provide compassionate, resident-centered care that upholds dignity and respect
• Collaborate with therapists, social workers, and other team members to enhance well-being
• Educate residents and families on health needs, medications, and care routines
