Licensed Practical Nurse

River Woods Exeter seeks compassionate RN/LPN nurses to join our Healthcare & Medical Services team in our continuing care retirement community. You'll provide resident-centered clinical care across independent living, assisted living, and nursing care, including assessments, medications, treatments, care planning, and coordination with the interdisciplinary team. Responsibilities

• Provide direct nursing care to residents in independent living, assisted living, and nursing care settings
• Complete ongoing assessments and document resident conditions and responses to treatment
• Administer medications and treatments safely and accurately per provider orders
• Develop, update, and implement individualized care plans with the interdisciplinary team
• Monitor residents for changes in status and communicate promptly with providers and families
• Supervise and support LNAs/aides, delegating tasks and ensuring quality of care
• Educate residents and families on health conditions, medications, and care plans
• Follow all infection prevention, safety, and regulatory standards (state and federal)
• Accurately document care in the electronic medical record and required logs
• Participate in care conferences, shift report, and quality improvement initiatives
