Assistant Social Worker

Nightingale Nursing & Rehab Center – Erie, PA

Nightingale Nursing & Rehab Center is seeking a compassionate and organized Part Time Assistant Social Worker to support our residents, families, and Social Services department. This role helps ensure smooth admissions, strong communication, and a positive resident experience within our skilled nursing facility.

• Assist with resident admissions, transfers, and discharges

• Support the completion of psychosocial assessments and care plan updates

• Communicate with residents, families, and outside agencies as needed

• Help coordinate services, appointments, and community resources

• Maintain accurate documentation in accordance with facility policies

• Support resident rights, advocacy efforts, and grievance resolution

• Participate in care plan meetings and interdisciplinary discussions

• Provide general administrative and organizational support to the Social Services Director

• Prior experience in social services, human services, or resident support preferred

• Experience in a skilled nursing facility or healthcare setting is a plus

• Strong communication, organization, and customer service skills

• Ability to maintain professionalism and confidentiality

• Compassionate, residentcentered approach

• High school diploma or equivalent required; relevant coursework or degree preferred

• Part Time

• Flexible hours; may include occasional weekends based on resident need
