LPN Care Coordinator Inn Horsham Center Jewish

The INN at The Horsham Center for Jewish Life is seeking new and experienced Licensed Practical Nurses (LPNs) to join our dedicated and compassionate care team in the role of Care Coordinator. This position functions in a charge-style capacity while supporting high-quality resident care in a warm, home-like environment.

• 7:00 AM – 3:00 PM
• 3:00 PM – 11:00 PM (every other weekend)
• PRN / Per Diem
• Schedule Options:

• Coordinate resident care and assume charge responsibilities for assigned shifts
• Provide oversight and guidance to Med Techs
• Monitor residents and report significant changes in condition (COS) to physicians, families, and facility staff
• Ensure nursing care aligns with individualized care plans, policies, and procedures
• Prepare and administer medications (PO, SC, IM) as ordered by physicians
• Maintain accurate narcotic documentation and medication records
• Properly return or dispose of discontinued medications
• Receive, document, and transcribe physician orders to MARs
• Communicate medication changes to pharmacy
• Complete daily wound documentation
• Report incidents and clinical concerns to Nurse Managers
• Complete all required documentation accurately and timely

• Active Licensed Practical Nurse (LPN) license in the State of Pennsylvania
• Experience in personal care or skilled nursing preferred
• Strong communication, organization, and leadership skills
• Ability to work independently and collaboratively
