Job Overview
Utilizing the nursing process, the Home Health Registered Nurse will manage and deliver comprehensive home health services, including assessments, interventions, and supportive care to clients primarily within their place of residence. Duties may also include telephone triage, crisis intervention, problem solving, patient/caregiver advocacy, and support, with an emphasis on avoiding hospitalization during off hours, weekends, or holidays. The role requires courteous interaction with patients, families, co‑workers, community resources, and physicians to facilitate safe and efficient patient care.
Location& Hours
Lowell, MA – Part‑time, 16 hours per week, weekdays only.
Minimum Qualifications- Massachusetts RN Licensure
- Valid state‑issued Driver’s License
- One year of acute medical/surgical nursing experience
- CPR Certification
- Assess and implement physical and psychosocial patient needs following standards of nursing practice and agency procedures.
- Identify learning needs of patients and families, adapt information delivery accordingly.
- Identify abuse or neglect and report per policy.
- Provide age‑appropriate care and demonstrate knowledge of growth and development across the lifespan.
- Use communication skills to understand patient reactions and responses to treatment.
- Engage patients in understanding and participation in treatment decisions.
- Collaborate with families and health‑care providers to plan care and discharge.
- Develop realistic written care plans with measurable goals in partnership with patients and caregivers.
- Conduct interdisciplinary assessments and evaluate care plans.
- Plan home death when appropriate with patients, families, and team members.
- Complete physicians’ orders, levels of care, and OASIS for all assigned patients per policy.
- Maintain awareness of community health needs and resources.
