Job Summary:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated team at Nightingale Nursing and Rehab Center in Erie, Pennsylvania. As an LPN, you will play a vital role in providing high-quality patient care and supporting our mission to deliver exceptional healthcare services to our residents.

Responsibilities:

• Provide direct patient care to residents in our long-term care facility, including administering medications, monitoring vital signs, and performing routine nursing procedures

• Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each resident's needs

• Conduct routine rounds to ensure resident safety and well-being, and address any concerns or issues promptly

• Document patient information accurately and thoroughly in electronic medical records

• Participate in quality improvement initiatives to enhance clinical outcomes and patient satisfaction

Qualifications:

• Current licensure as a Licensed Practical Nurse (LPN) in the State of Pennsylvania

• High school diploma or equivalent required; post-secondary education in nursing or related field preferred
