Social Worker – Full Time

Neffsville Nursing & Rehab is seeking a full-time Social Worker to join our compassionate team in Lancaster, PA. In this role, you will serve as a key advocate for residents—supporting their emotional well-being, guiding families, and helping ensure a smooth transition into and throughout their stay with us.

• Complete social assessments and care plans addressing residents’ psychosocial needs with realistic, actionable goals

• Participate actively in pre-admission and admission processes

• Provide compassionate support to residents and families during adjustment, crisis, or periods of heightened need

• Document behavioral and psychosocial updates in accordance with facility and regulatory requirements

• Lead individual and group support as needed

• Develop and grow volunteer programs tailored to resident needs

• Orient new residents and families to the center to ensure a smooth transition

• Bachelor’s degree in Social Work or related field preferred

• Knowledge of long‑term care social services practices, regulations, and guidelines

• Strong communication, empathy, and independent problem‑solving skills
