Licensed Practical Nurse

Christian Park Healthcare Center is seeking a compassionate Licensed Practical Nurse to provide resident-centered care in our skilled nursing and rehabilitation facility. The LPN will deliver medications and treatments, monitor resident conditions, document care, and collaborate with RNs, physicians, therapists, and families to support individualized care plans. This role supports short-term rehab, long-term care, and memory care residents while promoting dignity, safety, and independence in a homelike environment that values teamwork, clinical excellence, and quality outcomes.

Responsibilities

• Administer medications and treatments per provider orders and facility policies
• Monitor, assess, and document resident conditions and vital signs
• Assist with development and implementation of individualized care plans
• Provide direct nursing care, including wound care, catheter care, and ADL support as needed
• Communicate effectively with residents, families, and interdisciplinary team members
• Report changes in resident status promptly to RNs and providers
• Maintain accurate, timely, and compliant clinical documentation
• Support infection control, safety, and quality improvement initiatives
• Supervise and support CNAs and other support staff within scope of practice
• Promote resident dignity, comfort, and independence in a homelike environment
