Job Responsibilities
  • Complete an initial assessment of patient and family to determine home care needs.
  • Provide a complete physical assessment and history of current and previous illness(es).
  • Regularly re-evaluate patient nursing needs.
  • Initiate the plan of care and make necessary revisions as patient status and needs change.
  • Use health assessment data to determine nursing diagnosis.
  • Develop a care plan that establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions, including the patient and family in the planning process.
  • Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician.
  • Counsel the patient and family in meeting nursing and related needs.
  • Provide health care instructions to the patient as appropriate per assessment and plan of care.
  • Identify discharge planning needs as part of the care plan development and implement prior to discharge of the patient.
  • Act as Case Manager when assigned by Clinical Supervisor/Nursing Supervisor and coordinate patient care for assigned caseload.
  • Prepare clinical notes and update the primary physician at least every sixty days.
  • Communicate with the physician regarding the patient’s needs and report any changes in the patient’s condition; obtain/receive physician’s orders as required.
  • Communicate with community health personnel to coordinate the care plan.
  • Participate in on-call duties as defined by the on-call policy.
  • Ensure that arrangements for equipment and other necessary items and services are available.
  • Instruct, supervise and evaluate home health aide care provided every two (2) weeks.
Qualifications
  • Registered Nurse with current licensure to practice professional nursing in the state.
  • Graduate of a National League for Nursing accredited school of nursing.
  • Current CPR certification.
  • Licensed driver with a vehicle that is insured in accordance with state/or organization requirements and is in good working order.
  • Minimum of two years experience, at least one of which is in the area of public health or home care nursing is preferred.
  • Self-directed and able to work with minimal supervision.
  • Excellent observation, problem solving, verbal and written communication skills; nursing skills per competency checklist.
  • Ability to organize and prioritize workload independently.
  • Management experience not required. Responsible for supervising home health aides.
  • Physical ability to walk or stand for prolonged periods, lift, position, or transfer patients and supplies, and perform needed nursing skills.
  • All Registered Nurses employed by Archer Health may qualify for this position after completing the Community-Based Palliative Care competency checklist and as determined by Archer Health Clinical Director.
  • Additional experience and/or certification in Palliative Care or Hospice is preferred.
