Program:
SD EMS Corps Wraparound Program aims to provide TAY with supportive services such as case management, monthly stipends, one‑time equipment purchases, and barrier reduction supports that promote engagement and successful completion of the EMS Corps Education and Training Program. This position will specifically support the EMS Corps North County/Palomar College cohort, assisting participants in obtaining EMT certification with the Local EMS Agency and transitioning into full-time employment within the EMS workforce.
RESPONSIBILITIESUnder the supervision of the Program Coordinator, the Case Manager is the primary point of contact, with the goal of creating an authentic relationship with program youth. To build trust and understanding with the youth, the Case Manager will create individualized service plans with the youth and meet with them several times a week at school, home, or in their community to ensure that they stay on track.
The Case Manager is responsible for equipping the youth with the resources and the stability to successfully complete their training, obtain certification, and secure employment. The Case Manager also works as part of the youth support team, including the Life Coach and Educators, to remove barriers and increase the likelihood of successful program completion. Services will be in keeping with agency goals, objectives, mission, and style and provided within a team framework.
- Performs outreach and recruitment of eligible applicants for participation by providing program presentations and meetings with agencies and entities serving the population.
- Collaborate with other partners and staff to support youth’s successful completion of EMS training.
- Maintain regular communication with assigned students at key points in the semester to include reminders about important dates, upcoming events, and processes to be followed.
- Ensure that youth are supported for a minimum of 3 months post-graduation to help youth navigate the challenges of a new job.
