Salary Range: $26.00 To $28.00 Annually

JOB SUMMARY

Work with community organizations, government agencies and property owners to secure living arrangements, negotiate rents, deposits, and move-in terms on behalf of clients. Manage lease agreements for short-term, transitional, and permanent housing.

RESPONSIBILITIES
  • Conduct weekly housing inspections, follow reporting requirements, and work with clients to remediate issues.
  • Provide case consultation, crisis intervention, discharge placement, and targeted case management services to assist clients in meeting basic needs and housing needs.
  • Complete accurate and timely documentation as contractually required and meet minimum requirements for direct billable client services.
  • Locate housing opportunities to meet short-term, transitional, and permanent housing needs and manage lease agreements.
  • Prepare subsidy applications, maintain client housing files, and ensure rent calculations are accurate.
  • Assist in monthly tenant rent collection, transport clients to housing locations and help them secure required documents for housing placement.
  • Conduct housing screenings and assessments utilizing the coordinated entry system required.
  • Shift coverage including part of the on‑call team.
  • Provide field‑based services as required by program and client need.
  • Maintain confidentiality of client’s care.
  • Participate in scheduled meetings.
  • Other relevant duties as assigned.
QUALIFICATIONS
  • Bachelor’s degree and 1 year experience or associate degree and 3 years' experience or 5 years' relevant experience. Verify this is required.
  • High School Diploma or GED required.
  • Valid CA driver’s license and current auto insurance per contract requirements required.
  • Knowledge of housing needs of individuals with behavioral health needs preferred.
  • Knowledge of Microsoft Office and EHR software preferred.
