Health RIGHT 360 is seeking a Case Manager I to coordinate care across outpatient offices, residential programs, and community settings. You will assist with intake, apply for Medi-Cal, and connect clients to benefits and services as part of a multidisciplinary team.
The role requires reliable transportation and supports sensitive populations through a trauma-informed, evidence-based approach. Union-eligible, full-time, with a 8:00am-4:30pm schedule.
