Summary

Responsible for utilizing the nursing process to provide direct patient care to an assigned group of patients during a shift. Responsible for prioritizing the delivery of direct nursing care using time and resources efficiently integrating a standards-based framework model.

• Acts as clinical resource by overseeing delivery of patient care and directs and delegates patient care assignments to home health aides and nursing staff based on acuity and staff capability.
• Maintains clinical competency in providing direct patient care, evaluates outcomes, consults with specialists as required, and adjusts nursing care processes to ensure optimal patient care.
• Coordinates patient care through collaboration with patient, family, all other disciplines, primary and secondary physicians, and community resources/programs.
• Demonstrates knowledge, philosophy and provides support to patients and families.
• Ensures patients understand rights, responsibilities, advanced directives and informed consent during admission.
• Assesses and directs patient and family teaching needs, proficient in use of return demonstration and teaching aids in the home setting.
• Maintains professional responsibility for knowledge of current nursing research, practice standards, guidelines, protocols and hospital policies to improve quality.
• Maintains proficiency in use of all equipment used in practice.
• Effectively communicates with and assists other members of the nursing team or other disciplines; actively participates in problem‑solving and conflict resolution to maintain team cohesiveness.
• Performs on‑call duties, triaging patients over phone and visiting when needed.
• Directs own educational needs for the top 10 ICD‑9 codes/diagnoses for diabetes, wounds/post‑surgical, arthritis, newborns, Coumadin dependency, malnutrition/nutrition, medication instruction, pain and symptom control/hospice care.
• Supervises home health aides in compliance with Medicare/Medicaid rules and regulations.
• Performs other functions as required.

• Degree from an accredited academic nursing program (conditional program approval may be granted by system CNO).
• Minimum ADN preparation.
• Licensure:
  State of New Mexico or Compact State Nursing License required.
• Required certification from AHA, ASHI, or Red Cross.
• BLS for all RN roles.
• ACLS applicable to Emergency Department, Critical Care, Cardiovascular Lab, GI Lab, Interventional Radiology, Progressive Care, Surgical Services Pre‑Op and Recovery Room.
• NRP applicable to Family Birthing Center, Labor and Delivery, Neonatal Intensive Care, Mother/Baby Care.
• PALS applicable to Children Services, Cardiovascular Lab, Emergency Department, GI Lab, Interventional Radiology, Surgical Services Pre‑Op and Recovery Room.

The compensation range for this role takes into account a wide range of factors, including but not limited to experience and training, internal equity, and other business and organizational needs.
