Outpatient Physical Therapist - 5x8.0 Schedule Job Description

The Outpatient Physical Therapist is responsible for evaluating assigned patients and developing, implementing, and modifying treatment plans according to the patients' needs, goals, and physician direction. This role includes the delegation and direction of all discipline‑specific therapy services for those patients, ensuring the design of functional treatment programs that facilitate effective community reintegration. The therapist serves as an educator for patients, families, students, and coworkers, while upholding professional standards and compliance with organizational codes of conduct.

• Complete comprehensive evaluations for assigned patients and develop appropriate treatment plans and goals that aim for discharge within the established length of stay.
• Document medical records clearly, concisely, accurately, and timely in compliance with state, federal, and clinic guidelines.
• Ensure care delivery is based on signed, valid physician orders and within insurance approvals.
• Participate in the student program by providing supervision and evaluations in a positive learning environment.
• Create an individualized professional development plan while pursuing ongoing education and professional activities.
• Adhere to the discipline‑specific State Practice Act and maintain licensure in good standing, completing mandatory continuing education.
• Contribute to performance improvement initiatives and provide feedback to enhance service quality.

• Degree in Physical Therapy from an accredited school.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure for Physical Therapy.
• Previous experience with Select Medical is highly desired.

This is a 5x8.0 schedule position with guaranteed hours.
