Role Overview :
The Advanced Practice Clinician (APC) conducts comprehensive, one-time health assessments for patients transitioning from hospitalization to skilled nursing facilities or back home, supporting care coordination and improving patient outcomes.
Key Responsibilities:- Perform and document assessments in compliance with CMS regulations
- Ensure accurate ICD
10 coding and complete documentation of diagnoses - Conduct initial medication reviews and support quality measures
- Educate patients and families, communicate findings to providers and staff
- Travel between care sites as needed and participate in training
- Coordinate care by discussing medical concerns during visits
- Contribute to quality improvement initiatives and maintain proactive communication
- Active, unrestricted licensure (NP or PA) in practicing state
- Relevant certifications (ANCC, AANP, NCCPA)
- Reliable transportation and ability to navigate varied environments
- Proficient in electronic medical records and documentation
- Preferred: 1+ years clinical experience in geriatric or long-term care, familiarity with chronic illness and complex patient needs
