Key Responsibilities

The Licensed Practical Nurse provides direct nursing care to residents, ensuring care follows federal, state, local, and community standards and guidelines.

  • Comply with community personnel, safety, and corporate policies and procedures.
  • Adhere to nursing department policies and procedures.
  • Report suspected resident abuse or neglect to the Executive Director, RCC, or Unit Manager immediately.
  • Maintain the confidentiality of resident and family information.
  • Report to work on time and as scheduled.
  • Assist in admission and discharge of residents.
  • Assist residents with daily hygiene care (bathing, oral, ear, eye, facial, hair, and nail care).
  • Change clothing and linens as needed.
  • Assist residents with bowel and bladder functions and specimen collection.
  • Transport residents to appointments, activities, and social programs.
  • Assist with lifting, turning, moving, positioning, and transporting residents.
  • Document changes in resident condition or behavior to the Charge Nurse; assist with end‑of‑life care and post‑mortem care.
  • Administer professional services such as catheterization, tube feedings, suction, dressing changes, colostomy and drainage bags, blood draws, etc., as required.
  • Perform routine lab tests, blood pressures, and other monitoring per the care plan.
  • Complete accident/incident reports and submit to the Director as required.
  • Perform administrative duties: complete medical forms, reports, evaluations, studies, charting, and recordkeeping at admission, transfer, and discharge.
  • Chart nursing notes in an informative and descriptive manner that reflects the care provided and resident response.
  • Report discrepancies in physician orders, diet changes, or charting errors to the Nurse Supervisor.
  • Develop and maintain a good working rapport with inter‑departmental personnel to ensure nursing services can be adequately maintained.
  • Review complaints and grievances from staff; report to the Nurse Supervisor as required.
  • Receive and give the nursing report upon reporting in and ending shift duty hours.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals per policies.
  • Monitor personnel to ensure safety regulation compliance in equipment and supplies use.
  • Recommend equipment and supply needs to the Nurse Supervisor.
  • Review care plans daily to ensure appropriate care is rendered.
  • Oversee resident needs, staff, and operations as needed.
