Position Details:

• Outpatient only Family Medicine or Internal Medicine

• Four-day work week for full-time physicians

• Hybrid work schedule for eligible candidates

• Established patient panel with significant growth opportunity

• Predominantly adult and geriatric patient population

• Average 18-20 patients per day

• Protected administrative time built into the schedule

• Gradual panel ramp-up process and onboarding

• Minimal phone-only call

• Strong multidisciplinary support including:

  • Onsite pharmacist

  • RN case manager

  • LPN care support

  • Social work resources

  • In-basket support teams

  • Team-based MA model
