a highly collaborative primary care team delivering relationship-based care to a diverse patient population across the lifespan. This opportunity is ideal for a Primary Care Physician who enjoys caring for older adults while practicing in a supportive, team-oriented environment focused on access, continuity, and population health.

Position Details:
* Outpatient-only Primary Care practice
* Four 10-hour days, five 8-hour days, or 4.5-day schedules available
* Hybrid work model available for eligible providers
* Established patient panel with significant growth opportunity

* Flexible appointment templates (15/30 or 20/40 minutes)
