About the Position

Lane County’s Community Health Centers is seeking a dedicated Bilingual Community Service Worker (CSW) to join the Dovetail Program — a team of Community Health Workers embedded within our federally qualified health center (FQHC) primary care clinics.

The Dovetail Program addresses health-related social needs that impact stability, access, and overall well‑being. We recognize that housing, food security, transportation, income, and social support directly influence health outcomes. Our work is grounded in relationship‑based, community‑centered approaches that honor lived experience and promote dignity, equity, and empowerment.

In this role, you will partner directly with patients to identify priorities related to unmet social needs and help them build practical skills, community connections, and sustainable support systems aligned with their goals.

You will
  • Assist patients in navigating community resources and public benefits
  • Support access to housing, food, transportation, behavioral health, and other essential services
  • Coordinate with primary care providers and multidisciplinary teams
  • Document and track services within structured systems
  • Serve as a trusted bridge between patients, clinics, and community supports

This position works across clinic and community settings and is a recognized member of the primary care team, with a focus on the non‑medical factors that shape health and well‑being.

The ideal candidate

We are seeking a compassionate, culturally responsive professional who:

  • Has experience in community‑based or human services roles (community health work, outreach, case navigation, resource coordination, etc.)
  • Is familiar with Oregon Health Authority (OHA) Community Health Worker core competencies and values
  • Demonstrates strong interpersonal and cross‑cultural communication skills
  • Thrives in collaborative, multidisciplinary environments
  • Brings a trauma‑informed and strengths‑based approach
  • Is comfortable documenting services in electronic health record (EHR) or structured systems
  • Is proficient in basic computer applications

* OHA Community Health Worker certification is a program standard for this role. Candidates who do not yet hold certification will be supported in obtaining it.

About the Division

Lane County provides affordable primary care for the whole family – infants, children, adolescents, adults, and seniors – as well as integrated behavioral health, alternative services such as acupuncture, and preventative dental services. At Community Health Centers, we focus on integrated care that values the whole person. Our services encompass not only treatment and prevention of illness, but also health education and behavioral health services.

At Lane County Community Health Centers, we have adopted a model of primary care called Patient Centered Medical Home. Our services are organized around care teams who work collaboratively to support our patients. Our integrated care teams include primary care providers, nurses, medical assistants, patient care coordinators and behavioral health specialists. This model of primary care prioritizes the patient, who has increased access to other services beyond general medical care at our clinics.

Our care teams also help patients reach their optimal health status by addressing social determinants of health barriers to care such as transportation, food insecurity, or housing.

Monday - Friday; 8:00am - 5:00pm.

Education
  • Equivalent to the completion of two years of college course work.
Experience
  • Two years of responsible community service experience.
  • Or an equivalent combination of experience and training that will demonstrate the required knowledge and abilities is qualifying.
