Home Health & Hospice Aide (HHA/CNA) – PRN – Albany, OR. $23.00 – $27.00/hour (DOE) + Mileage Reimbursement.
What You’ll Do- Assist patients with personal care, including bathing, grooming, hygiene, and dressing.
- Support toileting, mobility, and transfers.
- Provide oral care, skin care, and repositioning to prevent breakdown.
- Assist with meal preparation, feeding, and light housekeeping.
- Perform range of motion exercises and activity support per plan of care.
- Observe and report changes in patient condition to the RN or therapist.
- Take and document vital signs (as trained and permitted).
- Maintain accurate, timely documentation of care provided.
- Work under the supervision of a registered nurse (RN) or therapist.
- Participate in the interdisciplinary care team (IDG).
- Attend case conferences and in‑service training.
- Current Home Health Aide (HHA) certification or Certified Nursing Assistant (CNA) in Oregon.
- Completion of a state‑approved training and competency program.
- Minimum age of 18 years.
- Current CPR certification.
- Valid driver’s license, reliable transportation, and auto insurance.
- Compassionate, patient‑centered mindset.
- Ability to work independently with minimal supervision.
- Strong communication and observation skills.
- Ability to follow care plans and document accurately.
- Professionalism, reliability, and empathy.
