Home Health and Hospice Aide | PRN | Albany

Home Health & Hospice Aide (HHA/CNA) – PRN – Albany, OR. $23.00 – $27.00/hour (DOE) + Mileage Reimbursement.

• Assist patients with personal care, including bathing, grooming, hygiene, and dressing.
• Support toileting, mobility, and transfers.
• Provide oral care, skin care, and repositioning to prevent breakdown.
• Assist with meal preparation, feeding, and light housekeeping.
• Perform range of motion exercises and activity support per plan of care.
• Observe and report changes in patient condition to the RN or therapist.
• Take and document vital signs (as trained and permitted).
• Maintain accurate, timely documentation of care provided.
• Work under the supervision of a registered nurse (RN) or therapist.
• Participate in the interdisciplinary care team (IDG).
• Attend case conferences and in‑service training.

• Current Home Health Aide (HHA) certification or Certified Nursing Assistant (CNA) in Oregon.
• Completion of a state‑approved training and competency program.
• Minimum age of 18 years.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.
• Compassionate, patient‑centered mindset.
• Ability to work independently with minimal supervision.
• Strong communication and observation skills.
• Ability to follow care plans and document accurately.
• Professionalism, reliability, and empathy.
