Overview

Licensed Clinical Social Worker (LCSW) to provide psychosocial assessments, discharge planning, and social casework for patients.

• Assessing patients' social, emotional, and financial needs
• Managing policies for patients' long- and short-term goals
• Collaborating with healthcare providers to understand social and emotional factors affecting patient care
• Documenting services in compliance with regulations and payer guidelines
• Provide psychosocial interventions, assessment, and advocacy
• Work in a fast-paced healthcare setting
• Support interdisciplinary planning and patient care

• Minimum 2 years of experience in social work
• Current LCSW credential (mandatory for travel/contract roles)
• Master’s degree from an accredited School of Social Work
• Excellent communication and decision-making skills
• Compassionate, detail-oriented, and attentive to patient safety
