The Opportunity:

Our client, one of the largest and most respected home health providers in the western U.S., is seeking a licensed Physical Therapist to deliver in-home rehabilitation services across Eugene and Salem, OR. This role combines clinical autonomy, patient-centered care, and flexible scheduling with the stability of a full-time position and full benefits package.

Key Responsibilities:

• Provide physical therapy services in patients homes based on physician-approved care plans
• Assess patient function, mobility, and pain levels to adjust treatment plans as needed
• Use therapeutic exercises, gait training, and mobility techniques to enhance recovery
• Educate patients and caregivers on safe practices and at-home therapy techniques
• Maintain accurate, compliant clinical documentation
• Collaborate with nurses, care coordinators, and physicians for integrated care delivery

Qualifications (Must-Have):

• Masters or Doctorate in Physical Therapy
• Active Physical Therapist License in Oregon
• Current CPR/BLS certification
• Minimum of 1 year of experience as a PT (Home health preferred)
• Valid drivers license and insured vehicle
• Willingness to travel throughout Eugene, Salem, and nearby areas
