Job Title
Physical Therapist

Schedule / Job TypeContract | Days | 13-Week Assignment
Key Responsibilities- Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting
- Develop and implement individualized physical therapy treatment plans
- Monitor and document patient progress toward established therapy goals
- Collaborate with interdisciplinary care team to coordinate patient care
- Educate patients and caregivers on therapeutic exercises and home programs
- Active Physical Therapist (PT) license in the state of Oregon or ability to obtain prior to start
- Graduate of an accredited Physical Therapy program
- Current BLS/CPR certification
- Experience working in a SNF or post-acute care setting
- Prior travel or contract therapy experience
- Familiarity with skilled nursing documentation and Medicare guidelines
- Opportunity to gain valuable experience in a skilled nursing environment on a 13-week contract assignment
- Supportive placement process with a dedicated recruiter to assist throughout your contract
