- Screen and assess patients for mental health and substance abuse disorders
- Facilitate patient engagement, treatment plans, and follow-up care
- Provide brief behavioral interventions using evidence-based techniques, including behavioral activation, problem-solving treatment, and motivational interviewing
- Support psychotropic medication management by monitoring adherence, side effects, and treatment effectiveness
- Provide health behavior and social determinants of health interventions
- Track treatment response and monitor patients in person, by video, or by telephone
- Educate patients about mental health and substance abuse disorders and treatment options
- Provide or facilitate referrals to evidence-based psychosocial treatments
- Participate in weekly caseload consultations with the psychiatric consultant and communicate recommendations to the medical provider
- Track follow-up and clinical outcomes using the EHR
- Document patient encounters, progress, and treatment recommendations in required systems
- Facilitate treatment plan changes for patients needing more intensive or specialized care
- Participate in process improvement initiatives
- Attend meetings and complete therapy primarily by video conference, with approximately monthly in-person clinic visits
- Meet monthly performance metrics determined by the clinical manager and clinical director
- Minimum of a master’s degree in social work, Psychology, Counseling, Rehabilitation, or another relevant field
- 3-5+ years of experience in behavioral health intervention clinical work, case management, and/or community care coordination
- Experience working with varied populations, including older adult Medicare populations with chronic medical and behavioral health challenges
- Active independent clinical license in Oregon required
- Eligible licenses include Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Psychologist
- Current enrollment in the state’s Medicaid and Medicare is preferred; eligibility for enrollment is required at minimum
- License-eligible candidates who have completed their board exam and are completing supervised clinical hours may apply
- Must work from the designated state of Oregon
- Stable high-speed internet
- Proficiency operating computer systems and using multiple software applications
- Proficiency with electronic medical records, scheduling systems, remote phone systems, and video systems preferred
Demonstrates expertise in mental health and substance abuse assessment, treatment planning, and evidence-based interventions. Proficient in managing psychotropic medications and tracking clinical outcomes while ensuring effective patient engagement and education.
