Ever Care Mobile Healthis seeking aPsychiatric Mental Health Nurse Practitioner (PMHNP) to join our growing team. We partner with residential and long-term care facilities to deliver comprehensive, evidence-based mental health services. Our integrated approach ensures continuity of care, allowing you to maximize your billable visits while making a lasting impact on patient outcomes.

- Conductpsychiatric evaluations, medication management, and therapy for residents in long-term care settings.
- Develop individualized treatment plansand collaborate with facility staff, primary care providers, and families.
- Maintain accurate and timely documentation in the electronic health record (EHR).
- Participate ininterdisciplinary team meetings to coordinate comprehensive patient care.
Compensation is competitive and aligned with industry standards, W2 or 1099 roles available
Qualifications- Active licensureas aPMHNP, PMHCNS or Psych-Certified PA.
- Licensure in Oregon
- Strong communication and collaboration skillsto effectively work with facility staff, families, and primary care teams.
- Consistent caseload– no no-shows or travel between individual patient homes.
