In-Home Health - Nurse Practitioner or Physician Assistant; Part Time - Niagara

Job Description

Signify Health, part of CVS Health®, is seeking a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations in patients’ homes. The visit lasts approximately 45 minutes and includes medication and medical history review, a physical evaluation, and, if ordered by the health plan, additional diagnostic tests such as diabetic eye exams or spirometry. Clinicians use an iPad and an intuitive clinical workflow for each evaluation.

They are not required to prescribe medicine, order lab tests, or alter current treatment regimens.

• Conduct virtual visits as required.
• Provide all Signify Health services as indicated, including diagnostic and preventive service products such as spirometry, diabetic eye exams, and bone density screening.

As an in‑home health clinician, your primary workplace is the private residence of our members. These environments are diverse and may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be able to comfortably and safely perform duties in such settings.

Base pay per In‑Home Health Evaluation (and associated visit) completed by a physician varies by experience, education, geography, and other relevant criteria. Position is eligible for additional reimbursement opportunities, including pay per completed Diagnostic and Preventative Services (DPS), as ordered.
