Licensed Practical Nurse (LPN) – On‑Call / After Hours
Location:
Albany, OR | Salary: $72,000 to $77,000 Annually DOE
Albany Home Health and Hospice is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our On‑Call/After Hours Team. This role provides skilled nursing care, after‑hours triage support, and home visits for home health and hospice patients throughout the Albany and Linn‑Benton areas.

- Respond to after‑hours patient calls and triage needs.
- Provide skilled nursing visits in patient homes.
- Complete admissions and follow‑up visits as needed.
- Administer medications and treatments as ordered.
- Assess patient conditions and communicate changes to providers.
- Educate patients and caregivers on care plans and symptom management.
- Maintain accurate and timely documentation.
- Collaborate with physicians, case managers, and interdisciplinary teams.
- Current Oregon LPN License required.
- Graduate of an accredited nursing program.
- Current CPR certification.
- 1–2 years of acute care nursing experience.
- Home Health or Hospice experience preferred.
- Valid driver’s license, reliable transportation, and auto insurance.
- Strong communication, assessment, and independent decision‑making skills.
